Szimultán végzett Phaco és PCL műtét iridocyclectómiával kombinálva

Esetismertetés: 53 éves erdélyi nőbeteg jobb oldali látásromlás miatt (V:0,06) jelentkezett
szemészeti vizsgálatra, ahol a kialakult szürkehályog mellett alul 5 és

7 óra között észlelt sugártesti és irisgyöki daganat miatt szemészeti onkológiai centrumba, a
Debreceni Egyetem Klinikai Központ Szemklinikára utalták. A kivizsgálás során UBM és
fotódokumentáció az elváltozást megerősítette, és a méretre való tekintettel nem Ru-
applikációt, hanem blokk excisiót indikáltunk szimultán phacoemulsificatio és PCL
beültetéssel.

A műtétet altatásban végeztük, elsőként lege artis phacoemulsificatio történt PCL beültetéssel,
majd a tumor elhelyezkedése miatt oldalról az egyenes izmokat fonalra véve a bulbust
elforgatva felezett scera lebenyt készítettünk. Iridocyclectomia alatt a systolés vérnyomást 80-
90 Hgmm közé szorítottuk le, majd kevés corpus ablálása utan a sebet 8/0-ás felszívódó
varratokkal zártuk, és exocryopexiát végeztünk preventív jelleggel. Sikeres műtét után
kialakult enyhe üvegtesti vérzés néhány nap alatt felszívódott, a posztoperatív 5. napon V:0,15
és a szemnyomás 14 Hgmm volt. Az elkészült szövettan épben eltávolított benignus Fuchs
féle adenomát igazolt.

Következtetés: A jó szövettani eredmény és posztoperatív látásélesség alapján úgy tűnik,
hogy a szimultán iridocyclectomia és szürkehályog ellenes műtét volt a megfelelő választás
ebben az esetben.

Simultaneous cataract surgery and iridocyclectomy

Case report: A 53-year-old woman from Transylvania presented for ophthalmic examination
due to visual impairment on the right eye (V: 0,06). Cataract and an iris-ciliary body tumor
located inferotemporally were diagnosed and she was referred to the Oncology outpatient care
of the University of Debrecen Department of Ophthalmology. Ultrasound biomicroscopy and
slit lamp photography confirmed the diagnosis. Due to the relatively small size of the tumor
surgical excision combined with phacoemulsification was suggested instead of brachytherapy.

Surgery was performed under general anesthesia. First, the phacoemulsification was
performed through a 12 o’clock incision, then due to the location of the tumor the inferior and
the temporal rectus muscles were taken to threads to rotate the eyeball and to be able to make
a scleral flap. During the iridocycletomy, the systolic blood pressure was brought down to 80-
90 Hgmm, open sky vitrectomy was made due to a small amount of vitreous prolapse and the
incision was closed by 8/0 absorbable sutures. The procedure was completed by a preventive
transscleral cryopexy. A mild vitreous hemorrhage developed after the surgery but re-absorbed
in a few days, and the visual acuity was 0,15 on the 5. postoperative day with an intraocular
pressure of 14 Hgmm. Histopathology revealed a benign lesion, Fuchs’ adenoma of the
choroid resected with tumor free margins.

Conclusion: According to the benign histopathologic result and the good visual outcome the
simultaneous iridocyclectomy and cataract surgery seemed to be the right choice in this case.
